Our on-call registered nurses have extensive experience and knowledge in HCS. The nursing staff has a profound understanding of group home and host home environments, allowing us to provide superior service to clients and Direct Care Professionals. Each client presents unique needs that require individualized care. Our dedicated nurses thoroughly assess each client’s functional capacity, medical condition, and treatment plan to create a tailored care strategy.

Our nurses provide comprehensive training to all staff members involved in the client’s care, ensuring they possess the knowledge and skills necessary to effectively and safely address the client’s specific needs.

Annually, our dedicated nurses conduct in-person Comprehensive Nursing Assessments and meticulously document any change in condition. Moreover, we provide all the needed corresponding documentation including AIMS, Barden Scale, Pneumonia scorecard, Client Specifics, Nursing Service Plans, etc. Throughout the years, our team has skillfully executed and finalized numerous Comprehensive Nursing Assessments for various providers statewide.

Theia Health provides a range of options for direct support professionals, including in-person training, synchronized audio-visual technology, and access to our dedicated online learning platform, Theia Learning Online.

Our general training covers essential courses such as Self-Administration of Medication Supervision (SAMS), Introduction to First Aid, Universal Precautions, Infection Control, Seizures, and Triage. Additionally, we offer advanced training on topics such as Diabetes, dietary considerations, and more.

Theia Health complies with the regulations outlined in the Texas Administrative Code, Chapters 224 & 225, and the Human Resource Code, Section 161.093. RN Delegation encompasses medication administration and treatments that are not exempt under the Human Resource Code, Section 161.093, as well as the initial medication dosage.

Ensuring that each client adheres to their prescribed medication regimen is crucial for our clients’ health and safety. Our dedicated nurses verify that every medication delivered to their home is accurately filled by the pharmacy according to physician orders. They also ensure that the Medication Administration Records align with the physician’s instructions and correspond to the medication blister pack or prescription bottle.

Every physician’s note or consultation undergoes thorough review and processing by our team of dedicated nurses. This involves meticulously examining the physician’s narrative, any new orders, labs, and diagnostic studies. In the event of new orders being prescribed, our nurses will expertly create an in-service and, if necessary, a Medication Administration Record (MAR). Additionally, Theia Health diligently inputs any follow-up appointments or studies that have been ordered by the physician into our team calendar for convenient viewing and tracking by the provider’s staff.

Our dedicated nurses recognize the uniqueness of each client and the varying levels of care required. They will create an Individualized Plan tailored to each individual, justifying the specific units necessary to deliver exceptional care

Our nursing staff is equipped to handle a diverse range of medical needs, exceeding the scope of care provided by unlicensed personnel. With their diverse backgrounds and skill sets, our nurses are prepared to address any medical requirements that our clients may have.

Theia Health meticulously tracks the journey of all medications prescribed by the physician and dispensed by the pharmacy. This monitoring begins from the point of receipt for review and continues until the medication is no longer required. Our nurses are fully equipped to appropriately dispose of any discontinued or unused medication in accordance with DEA regulations.

Host Home providers often lack sufficient nursing support, often receiving support only in crisis situations. Theia Health takes a proactive approach to ensure that host home providers receive the necessary support and follow-up nursing care to prevent crises.

Our nurses maintain regular monthly contact with the host home providers to inquire about the client’s well-being, recent injuries, falls, doctor’s appointments, Emergency Department or Urgent Care visits, as well as any new or changes in orders. This approach allows us to stay informed and provide timely assistance as needed.
